Peaks & Bounds now accepts Medicaid for outpatient occupational therapy.
We also offer private pay with a sliding scale.

$1,000 for a 10-session package
If Using Medicaid
If Using Private Pay
Complete the OT Referral/Intake Form, and we’ll reach out
to set up your sessions.
1. Ask your Primary Care Provider
for an OT referral.
2. Have the PCP fax the referral to
970-340-2488.
3. Once we receive both your OT Referral/Intake form and the
PCP referral, we’ll contact you to schedule.
